Please ask your patient’s parent/caregiver to complete our brief pre consult questionnaire. We will then contact your patient’s parent/caregiver to schedule their free pre-consult telephone appointment.
If you are a parent:
Please complete our brief pre-consult questionnaire. We will then contact you to schedule your free pre-consult telephone appointment. During this telephone call, we will review your pre-consult questionnaire and gather additional information regarding your child’s individual needs/concerns – to ensure “appropriateness of fit” (aka that the type of evaluation your child needs is within her scope of practice & expertise).
OKC DBA will be considered an out of network provider with all forms of insurance. We will NOT file insurance claims on a patient’s behalf nor will we conduct prior authorizations. However, we will gladly provide you with the necessary documentation to file an insurance claim on your own, if your insurance allows you to do so. Your insurance plan may reimburse you directly. Please contact your insurance company to determine your benefit(s) and the process required for your to file a claim. Please note, some insurance plans require a referral for reimbursement. It is your responsibility to determine if a referral is necessary. To avoid lack of reimbursement, obtaining a referral is strongly recommended.
We accept all major credit cards, debit cards, and cash if paid with exact change. We do not keep cash at our office. Checks/cashier’s checks are not accepted.
Please note:
50% of your “good faith estimate” will be collected when you schedule your child’s appointment.
The remaining 50% of your “good faith estimate” will need to be paid prior to your child’s appointment.
Any additional fees (for additional appointments and/or additional time spent reviewing outside records and/or documenting your child’s report) will be billed after Dr. Allen has completed your child’s comprehensive. Developmental & Behavioral Evaluation Report. Payment of any additional fees will need to be made in full prior to Dr. Allen’s release of your child’s comprehensive Developmental & Behavioral Evaluation Report.
Subsequent to your child’s initial consultation, any task that requires more than 10 minutes of time to complete (example: phone calls, reviewing and/or scoring additional paperwork or rating scales, placing orders, or completing additional documentation on your child’s behalf) may be billed based on the amount of time Dr. Allen spent completing these tasks (at a rate of $250/hour, to be billed in 10-minute increments).
Due to the high volume of patients who require a Developmental & Behavioral Evaluation, and the amount of time we spend completing each Developmental & Behavioral Evaluation, please note that we have a strict Appointment Reschedule, Cancellation & Missed Appointment Policy.
Reschedule 2 Weeks Out: If you reschedule your child’s appointment more than 2 weeks (11+ business days not counting weekends or holidays) prior to their scheduled appointment time, our policy is to allow your child’s appointment to be rescheduled without further charge/penalty. All payments already collected at the time of your child’s initial appointment scheduling will be applied to their rescheduled appointment. No further charge/penalty will apply.
Reschedule 5-10 Business Days Out: If you reschedule your child’s appointment within 5 – 10 business days (not counting weekends or holidays) prior to their scheduled appointment time, our policy is to allow your child’s appointment to be rescheduled, however, you will be charged a late rescheduled appointment fee of $250. All payments already collected at the time of your child’s initial appointment scheduling will be applied to their rescheduled appointment, however, the penalty fee will not be applied to their rescheduled appointment.
Reschedule 3 Business Days Out: If you reschedule your child’s appointment more than 3 business days (72+ hours not counting weekends or holidays) prior to their scheduled appointment time, due to a verifiable emergency, our policy is to allow your child’s appointment to be rescheduled without further charge/penalty. All payments already collected at the time of your child’s initial appointment scheduling will be applied to their rescheduled appointment. No further charge/penalty will apply.
Cancel 2 Weeks Out: If you cancel your child’s appointment more than 2 weeks (11+ business days not counting weekends or holidays) prior to their scheduled appointment time, our policy is that you will forfeit 50% of the payment already collected at the time of your child’s initial appointment scheduling. The remaining 50% will be refunded to you. No further charge/penalty will apply.
Cancel Less Than 2 Weeks Out: If you cancel your child’s appointment within the 2 weeks (10 business days not counting weekends or holidays) of their scheduled appointment time, our policy is that you will forfeit all payments already collected at the time of your child’s initial appointment scheduling. In addition, the appointment will be treated as a missed appointment and you will be billed the remaining balance of the “good faith estimate” that we provided to you at the time of your child’s initial appointment scheduling.
Missed Appointment: If you miss your child’s appointment, our policy is that you will forfeit all payments already collected at the time of your child’s initial appointment scheduling. In addition, you will be billed the remaining balance of the “good faith estimate” that we provided to you at the time of your initial appointment scheduling.
Your appointment time is reserved for you. Please help us to serve you better by keeping all scheduled appointments and by arriving on time for all appointments.

Building emotional regulation skills
Coordinating with school or therapists
